Copa Airlines Flight 201, a Boeing 737-200 was a flight between Panama City and Santiago de Cali, Colombia on June 6, 1992 with 47 people on board.
The Crash:
The airplane took off from Panama City at 8:37pm and 10 minutes later, ATC had notified the aircraft of bad weather in the area. At about 8:54pm, pilots had notified ATC that there were problems with the plane and had requested to turn back to the airport. In a short time later, the plane started to pitch down by 80 degrees and started to spin uncontrollably. The pilots tried to recover the jet from the dive, but when the plane reached 10,000 feet it exceeded the speed of sound, broke up in flames and crashed into the Darien Gap jungle. All on board were killed in the disaster. The crash was caused by the failed attitude indicator which was inoperative due to a short circuit. The pilots then believed they were banking left, when the aircraft was really banking right. It was also discovered that the backup indicator was still working, but the pilots didn't notice this due to poor cross checking procedures.
